Vital Records

Vital Records provide accurate data concerning life events, such as birth certificates and death certificates. The state of Georgia maintains vital records filed from 1919 to the present.

Clayton County Health District is responsible for filing, preserving, protecting, and issuing certified copies of all Georgia births.

Please be advised:  While we are now able to issue death certificates for events that occur within the state of Georgia, we cannot guarantee immediate issuance of the certificate if the event occurred in another state.

How to Request a Vital Record

By Mail
Pay by money order or cashier’s check to Clayton County Health District. DO NOT SEND CASH in the mail. Include a clear, readable copy of your photo ID.

Online
Clayton County does not accept online orders. However, we have partnered with an independent company where you can process online requests: VitalChek. There is an additional fee between $2-16 for using this service. VitalChek accepts all major credit cards.

In Person
You may request a birth certificate in person at our Forest Park Annex, Monday through Friday 8:00 a.m. – 4:00 p.m. Please bring the appropriate fee and a photo ID. We accept cash, Visa, Mastercard and money order. We DO NOT ACCEPT American Express checks or personal checks.

Fees

Search fee* (includes one certified copy, if the record is found on file): $25

Additional copies of certified record (ordered at the same time): $5

*Georgia law requires that a search fee is received before a certified copy of a birth certificate can be issued. Payment of the fee is necessary at the time of service, and search fees are non-refundable.
